What happened
Smith+Nephew plc (NYSE: SNN) cut its full-year trading profit growth forecast to around 4% from around 8% in a third-quarter update on Nov. 4. The company tied the cut to a lower revenue outlook, citing US hips and knees in the fourth quarter and market conditions.
Underlying revenue growth is now expected at around 3%, down from around 4%. Trading profit is seen at around $1.25 billion, down from around $1.3 billion. Free cash flow is expected at around $750 million, down from around $800 million. Trading cash conversion stays at around 95%, in line with original guidance. Adjusted return on invested capital is expected at around 9%, down from more than 10%.
The quarter held up better than the outlook. Q3 revenue was $1.55 billion, up from $1.50 billion a year earlier, a 3.4% rise on both an underlying and reported basis. Sports Medicine ENT grew 7.9% underlying to $507 million. US Knee Implants revenue fell 7.3% and US Hip Implants revenue fell 1.0%.

Why it matters
OptimistFi has argued that mid-single-digit growth and better margins drive the company's cash returns. The new guidance puts full-year revenue growth below that pace, so the case is mixed. Q3 revenue added $50 million to the year-earlier total.
Some supports remain. Guidance still has trading profit growing faster than revenue, and cash conversion is unchanged. CEO Deepak Nath said the company remains confident in profit growth ahead of revenue growth.
The caveat is US Orthopaedics. Nath said slower growth there is expected to continue through the fourth quarter. The Board has also been weighing strategic options for Orthopaedics. Smith+Nephew said it approached Johnson & Johnson earlier in the year, that talks took place and that they have now ended.

What's next
Smith+Nephew will report fourth-quarter and full-year 2026 results on March 1, 2027. Delivery at the lowered guidance would steady the case. Another cut tied to US hips and knees would weaken it.
The $500 million buyback, announced May 6, has $250 million completed. It is due to finish within twelve months of that date.
